More and more reports have called them revolutionary, with one headline even proclaiming these medications as “the most important drug breakthrough ever.” As with any so-called “miracle drugs,” misinformation is common and it’s easy to get swept up in the excitement.

Q: Many people have had significant success losing weight with GLP-1 medications, often citing their experience as life-changing. Does taking a GLP-1 drug guarantee significant weight loss?

While GLP-1 receptor agonists, like semaglutide (found in medications such as Wegovy® and Ozempic®), are associated with weight loss, individual responses can vary greatly. In clinical trials, many patients do experience weight loss, but it is not guaranteed, and the amount of weight loss can differ from person to person. Studies, such as the STEP 4 trial, have shown that while some individuals achieve substantial weight reduction, others may see more modest results due to a combination of factors, including baseline metabolic health, lifestyle habits, and adherence to dietary recommendations.

Q: So, how do GLP-1 drugs work in conjunction with an exercise routine? Is there any specific type of exercise that is shown to be beneficial with these drugs?

GLP-1 receptor agonists like semaglutide effectively control appetite and regulate blood sugar, but exercise enhances these benefits. A 2021 New England Journal of Medicine study found that combining semaglutide with lifestyle interventions, including exercise, led to an average 14.9% weight loss over 68 weeks—significantly more than lifestyle changes alone. Aerobic and resistance exercises improve metabolic health, insulin sensitivity, and body composition. The Obesity Medicine Association recommends 150-300 minutes of moderate aerobic activity or 75-150 minutes of vigorous exercise weekly, plus resistance training twice per week, for sustainable weight management and overall wellness.

Q: These can be powerful drugs! How often are side effects severe or unmanageable? What steps can be taken to manage common side effects?

Most people taking GLP-1 medications experience mild to moderate side effects, with nausea, vomiting, and gastrointestinal discomfort being the most common. However, severe side effects are rare, and most side effects can be managed by starting at a lower dose and gradually increasing as tolerated. It’s important for patients to follow their healthcare provider’s dosing recommendations and discuss any persistent side effects, as there are often adjustments or supportive treatments that can help manage these symptoms. Staying hydrated, eating smaller meals, and avoiding rich or fatty foods can also help minimize discomfort.

Q: What is the difference between compounded semaglutide and brand name drugs? Are they safe?

While compounded semaglutide contains the same active ingredient as brand-name drugs like Wegovy®, there are important differences. Compounded medications are prepared by state-regulated pharmacies that follow strict safety protocols but are not FDA-approved, meaning they haven’t undergone the same rigorous testing for efficacy and safety as brand-name medications. These formulations can offer flexibility in dosing and may be available in forms like oral or sublingual, making them an accessible option for some patients. However, it’s essential to consult with a healthcare provider to determine whether a compounded or FDA-approved version of semaglutide is best suited to your needs.
