STI Medication Side Effects and Warnings

DoxyPEP

DoxyPEP General Warning Disclosure

The CDC has endorsed DoxyPEP for preventing bacterial STIs as an STD prevention strategy. Current efficacy data on Doxy-PEP applies to gay, bisexual, MSM, and transgender women. Current research on DoxyPEP conducted with patients assigned female at birth is limited. It’s recommended to have baseline screening and follow up screening for bacterial STIs every 3–6 months at all sites of potential exposure. The FDA recommends periodic monitoring of liver and kidney function, as well as blood counts, during prolonged therapy with Doxycycline. If you are taking DoxyPEP frequently, it’s important to speak with your primary care provider for additional oversight and monitoring.

DoxyPEP Side Effects Disclosure

The most common doxycycline side effects include nausea and vomiting, upset stomach, loss of appetite, mild diarrhea, skin rash or itching, darkened skin color, and vaginal itching or discharge.

Serious side effects include:

  • flu-like symptoms;
  • red or purple rash on the skin that spreads and blisters or peels;
  • severe stomach pain, diarrhea that is watery or bloody;
  • throat irritation, trouble swallowing;
  • chest pain, irregular heart rhythm, feeling short of breath;
  • little or no urination;
  • low white blood cell counts - fever, chills, swollen glands, body aches, weakness, pale skin, easy bruising or bleeding;
  • severe headaches, ringing in your ears, dizziness, nausea, vision problems, pain behind your eyes;
  • loss of appetite, upper stomach pain (that may spread to your back), tiredness, nausea or vomiting, fast heart rate, dark urine, jaundice (yellowing of the skin or eyes).

Warnings

Cefixime

Cefixime General Warning Disclosure

Common side effects of cefixime may include:

  • Indigestion
  • Stomach pain
  • Nausea
  • Vomiting
  • Vaginal itching or discharge.

Serious side effects include:

  • severe stomach pain, diarrhea that is watery or bloody;
  • jaundice (yellowing of the skin or eyes);
  • pale or yellowed skin, dark colored urine, confusion or weakness;
  • a seizure (convulsions);
  • low blood cell counts--sudden weakness or ill feeling, fever, chills, flu-like symptoms, swollen gums, mouth sores, skin sores, rapid heart rate, pale skin, easy bruising, unusual bleeding, feeling light-headed;
  • kidney problems--little or no urination, painful or difficult urination, swelling in your feet or ankles, feeling tired or short of breath; or
  • severe skin reaction--fever, sore throat, swelling in your face or tongue, burning in your eyes, skin pain followed by a red or purple skin rash that spreads (especially in the face or upper body) and causes blistering and peeling.

Warnings

You should not take cefixime if you are allergic to cefixime, or to similar antibiotics, such as Ceftin, Cefzil, Keflex, Omnicef, and others. Tell your doctor if you are allergic to penicillins.


Metronidazole

Metronidazole General Warning Disclosure

Common side effects of metronidazole include:

  • Diarrhea
  • Nausea
  • Headache
  • Pain in the upper abdomen and abdominal cramping
  • Vomiting
  • Weight loss
  • Constipation

Serious side effects include:

  • Nervous system problems, including brain disorder (encephalopathy), inflammation of the brain and spinal cord membranes (aseptic meningitis), numbness or tingling in the hands or feet (peripheral neuropathy), and seizures (convulsions). Tell your healthcare provider right away if you have any nervous system problems while taking metronidazole
  • Worsening yeast infection (candidiasis) symptoms in people with a known yeast infection or a yeast infection they were not aware of
  • Low white blood cell count (leukopenia) in people with a history of blood problems. This can affect how well the body fights infection.
  • Abnormal heart rhythms (QT prolongation) may also occur. Metronidazole can cause life-threatening liver problems in people with Cockayne syndrome. If you have this condition, stop taking this medicine and contact your doctor if you have signs of liver failure--nausea, stomach pain (upper right side), dark urine, clay-colored stools, or jaundice (yellowing of the skin or eyes).

Warnings:

To reduce the development of drug-resistant bacteria, do not use metronidazole to treat any condition that has not been checked by your doctor. This medicine will not treat a viral infection such as the common cold or flu. Metronidazole has been shown to be carcinogenic in rats and mice and unnecessary use should be avoided.

Do not use metronidazole if you recently drank alcohol, or have taken disulfiram (Antabuse) within the past 2 weeks. Do not drink alcohol or consume foods or medicines that contain propylene glycol while you are taking metronidazole and for at least 3 days after you stop taking it.

Seizures and other nervous system abnormalities have been reported in patients treated with metronidazole. You should stop using this medicine immediately if you experience any neurological symptoms such as seizures, headaches, visual changes, weakness, numbness, or tingling.

Common Macrobid side effects may include: headache, dizziness; gas, upset stomach; mild diarrhea; or vaginal itching or discharge.

  • sudden chest pain or discomfort, wheezing, dry cough or hack;
  • new or worsening cough, trouble breathing;
  • fever, chills, body aches, tiredness, unexplained weight loss;
  • numbness, tingling, or pain in your hands or feet;
  • liver problems--nausea, upper stomach pain, itching, tired feeling, loss of appetite, dark urine, clay-colored stools, jaundice (yellowing of the skin or eyes); or
  • lupus-like syndrome--joint pain or swelling with fever, swollen glands, muscle aches, chest pain, vomiting, unusual thoughts or behavior, and patchy skin color.
  • Serious side effects may be more likely in older adults and those who are ill or debilitated.

You should not take Macrobid if you have severe kidney disease, complicated urinary tract infection, or a history of jaundice or liver problems caused by nitrofurantoin. Do not take Macrobid if you are in the last 2 to 4 weeks of pregnancy.


Bactrim

Common Bactrim side effects are: nausea, vomiting, loss of appetite, and skin rash.

Serious side effects include: severe stomach pain, diarrhea that is watery or bloody (even if it occurs months after your last dose); any skin rash, no matter how mild; yellowing of your skin or eyes; a seizure; new or unusual joint pain; increased or decreased urination; swelling, bruising, or irritation around the IV needle; increased thirst, dry mouth, fruity breath odor; new or worsening cough, fever, trouble breathing; high blood potassium - nausea, weakness, tingly feeling, chest pain, irregular heartbeats, loss of movement; low blood sodium - headache, confusion, problems with thinking or memory, weakness, feeling unsteady; or low blood cell counts - fever, chills, mouth sores, skin sores, easy bruising, unusual bleeding, pale skin, cold hands and feet, feeling light-headed or short of breath.

Warnings

You should not use this medicine if you have severe liver disease, kidney disease that is not being monitored, anemia caused by folic acid deficiency, if you take dofetilide, or if you have had low platelets caused by using trimethoprim or a sulfa drug.

You should not take this medicine if you are pregnant or breastfeeding.


Ciprofloxacin

Ciprofloxacin General Warning Disclosure

Common ciprofloxacin side effects may include:

  • nausea, vomiting, diarrhea, stomach pain
  • headache; or
  • abnormal liver function tests.

Serious side effects include:

  • severe stomach pain, diarrhea that is watery or bloody;
  • fast or pounding heartbeats, fluttering in your chest, shortness of breath, and sudden dizziness (like you might pass out);
  • any skin rash, no matter how mild;
  • muscle weakness, breathing problems;
  • little or no urination;
  • jaundice (yellowing of the skin or eyes); or
  • increased pressure inside the skull - severe headaches, ringing in your ears, dizziness, nausea, vision problems, pain behind your eyes.

Warnings:

Stop using ciprofloxacin and call your doctor at once if you have: headache, hunger, irritability, numbness, tingling, burning pain, confusion, agitation, paranoia, problems with memory or concentration, thoughts of suicide, or sudden pain or movement problems in any of your joints.

In rare cases, ciprofloxacin may cause damage to your aorta, which could lead to dangerous bleeding or death. Get emergency medical help if you have severe and constant pain in your chest, stomach, or back.

Do not take ciprofloxacin with dairy products such as milk or yogurt, or with calcium-fortified juice. You may eat or drink these products with your meals, but do not use them alone when taking this medicine.

Antibiotic medicines can cause diarrhea, which may be a sign of a new infection. If you have diarrhea that is watery or bloody, call your doctor before using anti-diarrhea medicine.

Ciprofloxacin could make you sunburn more easily. Avoid sunlight or tanning beds. Wear protective clothing and use sunscreen (SPF 30 or higher) when you are outdoors. Tell your doctor if you have severe burning, redness, itching, rash, or swelling after being in the sun.


Augmentin

You should not use Augmentin if you have severe kidney disease, if you have had liver problems or jaundice while taking amoxicillin and clavulanate potassium, or if you are allergic to any penicillin or cephalosporin antibiotic, such as Amoxil, Ceftin, Cefzil, Moxatag, Omnicef, and others.

Do not use while pregnant or breastfeeding.

Augmentin can make birth control pills less effective. Ask your doctor about using a non-hormone method of birth control (such as a condom, diaphragm, spermicide) to prevent pregnancy while taking amoxicillin and clavulanate potassium.


Pyridium

Common Pyridium side effects may include:

  • headache;

  • upset stomach.

  • little or no urinating;
  • swelling, rapid weight gain;
  • confusion, loss of appetite, pain in your side or lower back;
  • fever, pale or yellowed skin, stomach pain, nausea and vomiting; or
  • blue or purple appearance of your skin.

Warnings:

Do not take Pyridium if you are allergic to phenazopyridine, or if you have kidney disease. Pyridium will treat the symptoms of a urinary tract infection, but this medication does not treat the actual infection. Take any antibiotic that your doctor prescribes to treat your infection. To avoid stomach upset, take this medicine with food.

Pyridium will most likely darken the color of your urine to an orange or red color. This is a normal effect and is not cause for alarm unless you have other symptoms such as pale or yellowed skin, fever, stomach pain, nausea, and vomiting. Darkened urine may also cause stains to your underwear, which may or may not be removed by laundering.

Pyridium can also permanently stain soft contact lenses, and you should not wear them while taking this medicine.

Do not use Pyridium for longer than 2 days unless your doctor has told you to. Stop taking this medicine and call your doctor at once if you have pale skin, fever, confusion, yellowing of your skin or eyes, increased thirst, swelling, or if you urinate less than usual or not at all.

Do not use while pregnant or breastfeeding.

Warning:

Clindamycin can cause diarrhea, which may range in severity from mild to fatal colitis. Diarrhea associated with clindamycin use is sometimes caused by an overgrowth of dangerous Clostridium difficile bacteria in the large intestine. Seniors especially should be monitored for diarrhea. If you have diarrhea that is watery or bloody, stop using clindamycin and call your doctor. Do not use anti-diarrhea medicine unless your doctor tells you to. Seek urgent medical attention.

Get emergency medical help if you have any signs of an allergic reaction to clindamycin: (hives, difficulty breathing, swelling in your face or throat) or a severe skin reaction (fever, sore throat, burning in your eyes, skin pain, red or purple skin rash that spreads and causes blistering and peeling).

Serious skin and other reactions can occur with clindamycin. Seek medical treatment if you have symptoms of a drug reaction such as skin rash, fever, swollen glands, flu-like symptoms, muscle aches, severe weakness, unusual bruising, or yellowing of your skin or eyes. This reaction may occur several weeks after starting clindamycin.

Yeast Infection Medication Side Effects and Warnings

Fluconazole

Fluconazole General Warning Disclosure

Common side effects of Fluconazole may include: headache, diarrhea, nausea or upset stomach, dizziness, stomach pain, changes in the way food tastes.

Serious side effects include:

  • Allergic reactions to fluconazole. These are rare, but they can be serious and may include anaphylaxis. If you cannot reach your doctor, go to the nearest hospital emergency room. Signs of an allergic reaction can include shortness of breath; coughing; wheezing; fever; chills; throbbing of the heart or ears; swelling of the eyelids, face, mouth, neck, or any other part of the body; or skin rash, hives, blisters or skin peeling.
  • Liver injury. Fluconazole has been associated with rare cases of serious liver toxicity, including death primarily in patients with serious underlying medical conditions including pre-existing liver disease. In some cases, there was no obvious relationship to the total daily dose, duration of therapy, sex, or patient age. Fluconazole liver toxicity has usually, but not always, been reversible once treatment has stopped. Tell your healthcare provider if you experience signs of liver problems which may include loss of appetite, stomach pain (upper right side), tiredness, itching, dark urine, clay-colored stools, or jaundice (yellowing of the skin or eyes).
  • Skin conditions, including exfoliative skin disorders that have resulted in death in patients with serious underlying diseases, have been reported. Patients with deep-seated fungal infections who develop rashes during treatment with fluconazole should be monitored closely by a healthcare provider. Tell your doctor right away if you get a skin rash or lesions while taking fluconazole.
  • Cases of reversible adrenal insufficiency have been reported with fluconazole. Tell your doctor if you experience chronic, or long-lasting fatigue, muscle weakness, loss of appetite, weight loss, or abdominal pain.

Contraception Medication Side Effects and Warnings

Ella

Ella General Warning Disclosure

Common side effects of Ella may include: headache; dizziness, tired feeling; nausea, stomach pain; or menstrual pain.

Warning:

Call your doctor at once or seek emergency medical attention if you have severe pain in your lower stomach (especially on only one side) 3 to 5 weeks after using ella. You should wait at least 5 days after taking ella before you start using hormonal birth control (birth control pills, injections, implants, skin patches, vaginal rings). Both ulipristal and hormonal birth control may be less effective when used at the same time. You may need to use a barrier form of birth control until your hormonal birth control takes effect

Do not use while pregnant or breastfeeding.

You must use the medicine within 5 days (120 hours) after unprotected sex.


Plan B

Yaz

Warning:

Do not use Yaz if you are pregnant or if you recently had a baby.

Taking Yaz can increase your risk of blood clots, stroke, or heart attack. Many medical conditions can increase your risk of blood clot, stroke, or heart attack while taking Yaz. Tell your doctor about any and all medical problems, hospitalizations, surgeries, and medications before taking Yaz. You are even more at risk if you have high blood pressure, diabetes, high cholesterol, or if you are overweight. Your risk of stroke or blood clot is highest during your first year of taking birth control pills. Your risk is also high when you restart birth control pills after not taking them for 4 weeks or longer. If you are immobile for any reason (after a surgery or during long travel) you may also be at increased risk.

Smoking can greatly increase your risk of blood clots, stroke, or heart attack. You should not take Yaz if you smoke and are over 35 years old.

Mifepristone

Mifepristone General Warning Disclosure

Common side effects of mifepristone when used to terminate a pregnancy are: feeling sick (nausea), feeling tired or weak, cramping, vaginal bleeding, fever/chills, vomiting, headache, diarrhea, dizziness

The combination of mifepristone and misoprostol often causes cramps, nausea, diarrhea, and other symptoms. Your healthcare provider may send you home with medicines for these symptoms.

Warning:

Serious and sometimes fatal infections and bleeding are rare when mifepristone is used to terminate a pregnancy, but it is important to know that these can happen. In certain circumstances, serious bacterial infections can present without symptoms typical of an infection, for example, without fever or without shaking, chills, or gastrointestinal symptoms.

Cramping and vaginal bleeding are expected with this treatment. Usually, these symptoms mean the treatment is working. But sometimes you may get cramping and bleeding but still be pregnant. Prolonged heavy bleeding may be a sign of an incomplete abortion or other complications and further medical or surgical intervention may be required. Contact your healthcare provider right away if you have:

Heavy bleeding, especially if you bleed enough to soak through two thick full-size sanitary pads per hour for two consecutive hours or if you are concerned about heavy bleeding. In about 1 out of 100 women, bleeding can be so heavy that it requires a surgical procedure (surgical aspiration or D&C) Abdominal pain, abdominal discomfort, or “feeling sick”, including weakness, nausea, vomiting, or diarrhea, with or without fever, more than 24 hours after taking mifepristone.
